The ASCP Group Accidental Death and Dismemberment Insurance Plan is ideal
for members who want protection against both accident-related injuries
and death in one affordable plan. The Family Plan allows you to cover
your spouse and dependent children as well, so that you can care for them
and help compensate for the loss of income if they are involved in a covered
accident.
As a member or registrant of
ASCP
under age 70 and a U.S. Resident residing in the U.S., you may apply for
coverage. Your legal spouse and unmarried dependent children ages 15 days
to 19 years (25 if a full-time student) are also eligible for coverage.
You may choose the plan that best fits your needs:
- Personal Plan
You may select an insurance amount (Principal
Sum) from $25,000 to $300,000 in units of $25,000 of personal or family
coverage.
- Family Plan
You may select an insurance amount (Principal
Sum) from $25,000 to $300,000 for yourself. Your spouse is automatically
insured for 40% of your insurance amount and each eligible child for
5%. If you have no children, your spouse is covered for 50% of your
insurance amount. Or if you are a single parent, each child is insured
for 10% of your insurance amount.

additional benefit is payable if the insured individual was positioned
in a seat protected by a properly functioning original, factory installed
airbag system that inflates on impact when the accident occurred.
—an
additional benefit is payable for an insured individual’s surviving
child(ren) if at the time of such accidental death, his or her child is
under age 13 and enrolled in a day care program. The benefit is paid each
year for each child who qualifies for the Day Care Benefit except that
no more than four Day Care Benefits will be paid for each such child.
—an
additional benefit is payable if at the time of the insured individual’s
accidental death, his or her insured dependent(s) is a student attending
an institution of higher learning, or if within 12 months of attending
such institution, provides proof of enrollment in such institution. The
benefit is payable each year for up to four consecutive years as long
as the dependent remains a full-time student.
—if
an insured individual dies outside the territorial limits of the state
or country of his or permanent residence, a Repatriation Benefit is payable
for (a) preparation of the body for burial or cremation; and (b) transportation
of the body to the place of burial or cremation.
—an
additional benefit is payable if an insured individual dies as a result
of injuries sustained in an automobile accident while traveling and properly
wearing a seat belt, provided death occurs within 90 days of the accident.

If an injury results in either death or dismemberment
within 365 days after the injury, your Plan pays:

The total benefit payable for all losses that
a person suffers as a result of one injury shall not exceed the full amount
of the Principal Sum for which that person was insured on the date of
the injury.
Only one benefit, the largest to which you
are entitled, is paid for all losses from the same accident. All coverage
will cease the date any Principle Sum are paid as a result of a covered
injury suffered by the member or registrant.
- A covered loss is a loss that a covered person
suffers, but only if such loss results from an accidental injury and
such: (a) loss occurs within 365 days of such injury; (b) injury occurs
while he or she is insured under the Policy; and (c) injury is the direct
result of the accident and is independent of disease or bodily infirmary.
- Loss
Means with regard to: (a) limbs, actual severance
through or above the wrist or ankle joints; (b) sight, speech or hearing,
entire and total and permanent loss; (c) thumb and index finger, actual
severance through or above the metacarpophalangeal joints; and (d) movement
of limbs, total and permanent paralysis.

Accidental
Death & Dismemberment Insurance rates
- Exclusions
The plan does not cover any loss resulting
from:
• Travel in or on or fall from any aircraft while the aircraft
is in flight unless you are traveling (a) solely as fare paying passenger
on a licensed, commercial, regularly scheduled, non-military
aircraft; (b) in a civil aircraft having a current and valid “Standard
Federal Aviation Agency Airworthiness Certificate” and piloted
by a person with a current and valid pilot’s certificate with
proper rating for the type of flight and aircraft nvolved; or (c) in
a transport type aircraft operated by Military Airlift Command (MAC)
or its successor organization of the United States or similar air service
in any other country.
• Participation in or incarceration resulting from the commission
of a felony, an illegal occupation or activity, an insurrection, terrorist
activity or a riot in a role other than as a victim.
• Service in the military, naval or air force of any country,
alliance or international organization or in a civilian unit which serves
such force.
• Suicide, an attempt at suicide or an intentionally self-inflicted
injury which occurs during an attempt at suicide or while intentionally
injuring yourself while you are sane or insane.
• Engagement in war, an act of war or an armed conflict which
involves the armed forces of one or more countries in a role other than
as a victim.
• Disease or bodily infirmity of mind or body; medical or surgical
treatment of such disease or bodily infirmity; or bacterial infections,
except infections which occur as a result of an accident cut or wound
or accidental ingestion of contaminated material.
• Legal or illegal use of drugs, intoxicants, narcotics, barbiturates,
or hallucinogenic agents, unless such use is prescribed by a doctor
or accidentally administered; legal intoxication.
• Medical, dental or surgical treatment unrelated to the accident
which would otherwise entitle you to benefits.
- Termination
You may renew your coverage until age 70 as
long as you remain a member or registrant of
ASCP
; continue to pay your premiums when due; do not begin active duty in
the armed forces; and the Group Policy remains in force. Your spouse’s
or dependents’ coverage or both will terminate when yours does,
or when your spouse attains age 70 and your dependent child attains
age 19 (25 if a full-time student) or when they no longer meet the eligibility
requirements; and the Group Policy remains in force; or their premiums
are not paid; or they begin active duty in the armed forces.
- Effective date of coverage
Your coverage will become effective on the
first day of the month after your enrollment form and first premium
payments are received by the administrator.
