Get up to $300 per day in benefits



 

 

You can apply for benefits from $20 up to $300 per day with this affordable plan.

Eligibility

As a member or registrant of ASCP in good standing, under age 70, and a U.S. Resident, you may apply for benefits from $20 a day up to $300 a day in units of $20. Your legal spouse (under age 70) and a U.S. Resident is also eligible to apply for an equal amount of coverage. Your eligible, unmarried dependent children under age 23 may enroll for coverage.

Choice of Plans

You may choose the plan that best fits your needs:

  • Member/Registrant Only
    You may select benefit amounts from $20 to $300 a day in units of $20.00.
  • Member/Registrant & Spouse
    You may select benefit amounts for you, your spouse and dependant children from $20 to $300 a day in units of $20.00.

Payment of Benefits

Benefits are paid directly to you (unless you instruct us differently) to be used in any way you wish. They can be applied to your hospital bill; used to cover additional costs; or used to meet your family’s regular, ongoing expenses while you are hospitalized. An additional benefit, which doubles your hospital income amount, is payable when the Insured is confined in an Intensive Care Unit or Cardiac Care Unit of a hospital for treatment of a sickness or injury up to 365 days.

Other important provisions of the plan

  • Pre-existing Condition Limitation
    During the first 12 months of coverage, losses incurred for Pre-Existing Conditions are not covered. A Pre-Existing Condition means any injury or sickness including pregnancy; diagnosed or undiagnosed, for which you have received medical care within the 12-month period prior to your coverage effective date or the date of an increase in coverage. During that time, benefits for all other accidents or illnesses will be paid under the policy provisions. You are urged to consider this limitation before dropping any coverage you may have until the waiting period is over.
  • General Exclusions
    The Policy does not cover: intentionally self-inflicted injuries, suicide or attempted suicide, whether sane or insane, (in Missouri or Colorado while sane); any loss caused or contributed to by war or act of war, whether war is declared or not; any loss which occurs while the Covered Person is in any of the armed forces, whether land, water or air of any country or international authority; pregnancy or childbirth, except Complications of Pregnancy; Confinement in a Veterans Administration or any other National Government owned or operated Hospital.
  • Drug Alcohol and Mental Nervous Limitation
    The benefit period for confinement for Mental or Nervous Disorders, alcoholism or drug abuse, is 30 days reduced to 60 days during your lifetime.
  • Terminaton
    You may renew your coverage until age 70 as long as you remain a member or registrant of ASCP continue to pay your premiums when due;and the Master Policy remains in force. Your spouse’s or dependents’ coverage or both will terminate when yours does, or when they no longer meet the eligibility requirements; or their premiums are not paid.
  • Effective date of coverage
    Your coverage will become effective on the first day of the month after your enrollment form is received by the administrator, provided payment is received when due and you are not hospitalized or disabled on that date. If you are, the effective date will be delayed until the first day of the month following the end of your hospital stay.
  • Definitions

    Hospital
    does not mean any institution or part thereof which is used primarily as:
    a) a nursing home, convalescent home, or skilled nursing facility;
    b) a place for rest, custodial care, or for the aged;
    c) a clinic; or
    d) a place for the treatment of mental illness, alcoholism, or drug addiction.

    Confinement means being an Inpatient in a Hospital due to Sickness or Injury.

    Successive Confinements:
    a) due to the same or related causes; and b) separated by less than 6 months; are part of the same Period of Confinement. A new Period of Confinement begins when the Covered Person is readmitted to a Hospital:
    a) for a new Sickness or Injury unrelated to the causes of a prior Confinement; or b) after he or she has been free of Hospital Confinement for 6 months or more.

    Injury/Injuries: Injury means bodily injury of a person resulting directly and independently of all other causes from an accident.
